LEVELS OF CARE SERVICE DESCRIPTIONS
 

SUPPORTIVE LIVING: Base Room and Board.   No personal care services needed.

3 meals/day and nutritious snacks; 24 hr./day support staffing; emergency call system; daily activities program; weekly housekeeping/linen service; cable television and all utilities (excluding telephone).

A person may receive ancillary services per choice:

            Medication Management = $275/month                      Shower or Bath = $10.00/each time
            Personal Laundry = $5.00/load                                   Extra Hskpg. = $5.00/each time
            Bed Making = $1.00/day                                              Treatment = $5.00/each time
            Wireless Call System = $25.00/mo.

ASSISTED LIVING:  Personal care services needed on a regular or intermittent basis.

All of the amenities of Supportive Living (+) assistance with activities of daily living   such as….dressing, grooming, hygiene, incontinence management, bathing; medication management; memory care; laundry, bed making, and wireless call system.

SPECIALIZED DEMENTIA CARE: “GRACE HALL” Personal care services needed on a consistent basis.

All of the amenities of Supportive Living (+) Assisted Living Services (+) Specialized Dementia Care programming including daily activities, specially trained staff, higher staffing ratio, and small, secured living area.

RATES AND PAYMENT OPTIONS:

Option A =       Monthly fee, no entrance fee required.

Options B=       A one time entrance fee of $20,000 ($30,000 for couple) receives a discount of $620/month ($992/month for couple) as long as he/she resides in the Home for the Aged (Supportive, Assisted Living, Specialized Care). 

* The d?uble room on Grace Hall for Specialized Dementia Care may be used for a couple (as long as one meets criteria for specialized dementia care and they prefer not to be separated) or will be used as a semi-private room for two unrelated persons.  The double room will not be available for one person exclusively.

Nursing Care:  Nursing care services needed on a 24 hour basis.

24hr Licensed Nursing Care; Medication management administered by a licensed professional; Daily living assistance; housekeeping; linen service. Private and Semi-Private rooms available.
